New patient policy

If you are a new patient, please complete the New patient registration form online or download, print, fill and bring for your appointment. Please bring your Medicare card and Pensioner concession card or DVA card.


We are a mixed billing practice. We offer bulk billing for patients 15 years and under, DVA gold card, DVA white card for eligible conditions and senior pensioners/concession card holders.
